Initial email to new cohort about DPS documentation

Hello all,

It was great to see you at orientation yesterday evening, and I’m looking forward to meeting you all in person soon!

I’m emailing today with details about the forms that need to be completed and sent to Durham Public Schools. The school system requires two forms to be completed and processed before student teachers can work in or visit one of their schools:

Consumer Report Notice – This form is required for DPS to start the background check process. As background checks vary in the length of time they take to complete, the sooner you can get this form back to me, the better.

Health Form – This form requires a signature from a health care provider. It can be completed by an urgent care or minute clinic type place if getting the paperwork back from your primary doctor is difficult. Fast Med Urgent Care is on Hope Valley Rd., The Target on 15-501 has a minute clinic on site. There are a number of other urgent care and clinic options in Durham.

Please email completed forms to me as soon as possible, and I will send them on to Durham Public Schools.
