Need DOC Team Social Work? (a.k.a. JAN AND ELISSA)

How to Refer: It's now as easy as 1-2

  1. Your first choice (and never the wrong answer) is to find Jan Dillard, LCSW or Elissa Nickolopolous, LCSW in the back hallway on the Lavender side. Or use Secure Chatin Epic (speech balloon in upper left corner) to ping them.
  2. If a patient has been coming to the DOC for a while, and especially if  dealing w/ a chronic issue, type "LCSW" in the Search box in the patient's chart and it should pull up prior Social Work notes, so you can double-check if the issue has been addressed previously (maybe even last visit).

    Note: EComm DOC Social Work has been discontinued.

Durham Ensure Program 

A new program from Durham Center for Senior Life provides Ensure to Durham residents 55+ the first Friday of every month! Recipients have to come to the DCSL offices at 406 Rigsbee and can receive up to 2 cases; all will be asked to sign up for a free DCSL membership. Pickup is from 10a-noon, and first-come, first-served based on priority (post-hospital or facility discharge w/ doctor's note; current DCSL day health participants; disabled seniors, etc.).

Duke  Caregiver Support Program

The Duke Caregiver Support Program offers free support around caregiver needs; you do not need to be a Duke Health patient to receive these services.

The program helps connect caregivers to verified service organizations and resources at Duke Health and in our local community, and provide guidance on home care and placing loved ones into long-term care communities, as well as emotional support and education.  Click here for more info.


Dementia Care Management

DCSL is starting a new program for individuals living with dementia or cognitive impairment and who live alone in Durham.
